Please complete fully and submit for processing. You will be contacted if additional information is required.

On the "Recipient's Preferences" line, please specify what items/products you believe the recipient would appreciate, known allergies (if any) as well as their favorite color. The latter is used as a source of reference to embellish their gift (florals, accessories, bow, etc.) 

Also, please do not forget to send a message to the recipient by completing the "Enclosure Card" field.
